Is there anything required in order to buy a new merit, other than having enough XP?

Only if it has a specific prerequisite, like Oracle. Otherwise, just the xp (3+3 for each other merit you have, minus certain exceptions)

So, to test my understanding, if you have 3 merits already, a 4th would cost 12 XP (3+(3*3)), is that correct?

What are the "certain exceptions"?

Does a human's free merit not increase the cost of all subsequent merits?

A human's free merit counts in all ways as a normal merit. So if you take Income as your free merit for being human and two games later you decide you want to buy Sorcerer, it will cost you 6xp.

The "certain exceptions" are a handful of merits that explicitly exclude themselves from the cost of subsequent merits:

Fomori
Ghoul
Kinfolk
Gifted Kinfolk
Sorcerer
